- Enter youngest age group 1st and assign # to team. Complete balance of the form.
NOTE: If there are 40 teams in the club, please ensure that teams are numbered 1-40 ascending in age.

- All artwork must be submitted electronically, preferably by e-mail. Dasport accepts practically all digital formats.
- Artwork should be e-mailed to info@dasportsoccer.com at least 6 weeks prior to the requested delivery date.
- Artwork should be a minimum of 300 dpi.
- Dasport will accept artwork in other forms but additional charges may apply.
Terms and Conditions of Program:
- Orders must be confirmed in writing. Lead times are required to ensure timely delivery.
- A 25% deposit is required at the time the order is placed.
- There are no minimum requirements on re-orders. Please note that most other companies do not permit re-orders at the same prices!
- A minimum order of 200 sets in at least 4 different colors is required to obtain set pricing. At the discretion of Dasport a surcharge of $2.50 per set applies for orders over 100 sets but less than 200 sets or orders for one color.
- If a Club cannot purchase the entire set, Dasport will in certain circumstances offer only shirts. Call for quote.
- SPONSORS – Dasport will print sponsor’s logos on jerseys for an additional charge. Please see dealer or call for details.
